If you are a new patient or have not seen us in more than 2 years, we ask that you print the following documents before your appointment and bring them completely filled out to your appointment.

Dental History
Medical History
HIPAA Notice of Privacy Practices

If you are unable to print these documents or if you forget them, please arrive 20 minutes before your scheduled appointment time and we will give you copies to fill out.

Also, please do not forget to bring your insurance card. We will need to make a copy of it and keep that on file.

If you have had x-rays taken within the past year, please bring them to your initial appointment or arrange to have them e-mailed to our office at It is impossible to provide you with a comprehensive exam without viewing any x-rays.

We always attempt to clean your teeth on the day of your initial visit to save you time; however, we cannot guarantee that there will be enough time. If you have a detailed medical or dental history or have not had your teeth cleaned in several years we may have to ask that your return another day to clean your teeth. Also, if you have gum disease you may require a deep cleaning (a procedure called Scaling and Root Planing) which would have to be re-scheduled for another day.
